Document arrange requirements
USCIS has some guideline about how you should send your documents to Chicago Lock box. However, you can find detailed information about document arrangement in here.
Each applicant in your household must have own AOS package, i.e. one AOS package in a single envelope. i.e. If you are married with no kids, you should have two envelopes for both you and your spouse.
Each AOS package should have these documents. Arranging these documents is also very important and USCIS lockbox will appreciate it (in rare case, your application may get reject if you follow wrong arrangement as they cannot find the documents)
- Cover letter
- Payment (Check or Money order or G-1450)
- eNotification form ( Form G-1145)
- Two identical photos
- Sealed I-693 medical
- Completed form 1-485
- DV Selection Letter
- 2NL from KCC
- DV fee Receipt
- Birth Certificate (followed by English translation)
- Passport Bio page ( and previous passports if applicable)
- Current Visa page on your passport (and any previous Visa pages)
- Most recent I-94
- Visa Stamp pages from your passport
- Current immigration or nonimmigration status ( i.e. I-20, DS 2019)
- Previous immigration or/and nonimmigration documents (if any)
- Proof of maintaining current status (i.e for F1, Class Enrollment verification)
- Last 3 years W2s (if available)
- Social Security card
- Tax transcript (last year, if available)
- Pay stubs (3 months, if available)
- High school diploma (and translation if required)
- Any documents you received from USCIS (if applicable)
- Marriage Certificate (for spouse package)
